You have decided to embark on your blissful journey. We here at Salem City Hall
want to make sure that your travel is as tension free as possible. Based on requirements
we have created the following instructions, which will guide you to prepare the
necessary forms to be submitted to the City Clerk’s office in order to comply with
the requirements for the Commonwealth. **Please note: You are applying for this
marriage intention with the City of Salem.

Fill up the following forms.
a. Notice of Intention form
b. Supplementary form
c. Worksheet form
Intention form - Please make sure that the following information is handy when filling
out the forms. All biographical information – Sometimes not readily available are
Mother/Father’s Surname of birth or Adoption. Make sure you have this information.

Venue - By this time you decided on your venue - Salem
City Hall, chapel or a venue of your choice. Check off the selected site. If city
hall is where you are getting married, almost all information is autofilled. If
the venue is other than the city hall, then provide name and address of venue.

Officiant - Provide officiant’s name and address and telephone
number.

Complete all 3 forms online and submit online to the attention of the City Clerk.
An email will be sent to you.

Once the City Clerk’s office accepts the application, both parties will be contacted
via phone or email. At that time, both parties will be instructed to come to Salem
City Hall to sign the paperwork and pay the $40.00 filing fee.
8.
Your marriage license will be ready for pick up 72 hours after the application has
been signed and paid for.
9.
You should proceed to get married within 60 days, or the license will expire.
10.
The Officiant must sign and certify the Marriage License and send license back to
Salem City Hall, and a Marriage Certificate will be mailed to you.
11.
You may request additional copies of your Marriage Certificate from Salem City Hall
